Remodeling in Persistent Atrial Fibrillation: Pathophysiology and Therapeutic Targets—A Systematic Review

نویسندگان

چکیده

Atrial fibrillation (AF) is characterized by disorganized rapid atrial electrical activity, which leads to impaired function, adverse hemodynamic effects, and increased thromboembolic risk. The paroxysmal forms of AF can be effectively treated with current pharmacological non-pharmacological modalities targeting the arrhythmia triggers. Persistent AF, however, more difficult treat due remodeling processes may become major factors in maintenance arrhythmia, rendering trigger-targeting treatment options less effective. We will systematically review recent findings development persistent including genetic, cellular, organ level, systemic processes. As remains most common sustained ongoing need find effective treatment, we also discuss potential

Gene Expression and Ultrastructural Remodeling in Persistent Atrial Fibrillation

Background:Atrial fibrillation (AF) has a tendency to become persistent over time and is known to induce atrial electrical, mechanical and ionic remodeling. However, the underlying mechanisms by which AF persists were not fully determined. The present study was carried out to assess alterations in the gene expression, including the oxidative stress-related genes of atrial myocardial cells in pa...

Left atrial wall stress distribution and its relationship to electrophysiologic remodeling in persistent atrial fibrillation.

BACKGROUND Atrial stretch causes remodeling that predisposes to atrial fibrillation. We tested the hypothesis that peaks in left atrial (LA) wall stress are associated with focal remodeling. METHODS AND RESULTS Nineteen patients underwent LA mapping before catheter ablation for persistent atrial fibrillation. Finite Element Analysis was used to predict wall stress distribution based on LA geo...

Reverse atrial electrical remodeling: a systematic review.

Atrial remodeling is a term introduced in 1995 to describe alterations in atrial structure or function. Atrial electrical remodeling is characterized by a reduction of refractory period and action potential duration, dispersion in refractoriness, and a reduction in conduction velocity of impulse propagation. Numerous animal and human studies have demonstrated that atrial electrical remodeling i...

Left atrial remodeling: Regional differences between paroxysmal and persistent atrial fibrillation

BACKGROUND The mechanisms underlying self-perpetuation of persistent atrial fibrillation (AF) are not well understood. To gain insight into these mechanisms, we conducted a study comparing left atrial (LA) electroanatomic maps obtained during sinus rhythm between patients with paroxysmal AF (PAF) and patients with persistent AF (PerAF). METHODS The study included 23 men with PAF (age, 56.3±12...

Atrial Fibrillation Atrial Fibrillation Pathophysiology Implications for Management

Atrial fibrillation (AF), the most common sustained cardiac arrhythmia, is an important contributor to population morbidity and mortality. An arrhythmia that is particularly common in the elderly, AF is growing in prevalence with the aging of the population. Our understanding of the basic mechanisms that govern AF occurrence and persistence has been increasing rapidly. This article reviews the ...
